Admission Information at Putnam Acres Care Center

At Putnam Acres Care Center we are dedicated to assuring that each admission to our facility is as seamless as possible. We realize that this is a stressful experience and that is why the dedicated team of caring professionals will work with you to assure that your needs are met. The staff will assist in facilitating admission by contacting the appropriate individuals or agencies to obtain all necessary medical and financial information required for admission.

For admission from the hospital, the Putnam Acres Care Center admissions coordinator will work with the hospital social worker to manage the admission process.

For admissions from home, the admissions coordinator will work with the family physician to arrange for completion of the paperwork required for admission. Once a date and time for admission has been scheduled, the admissions coordinator will arrange a convenient time with the resident, family member, or responsible party to complete the necessary admission forms.

On the day of admission, you are asked to bring the following items:
  • Medicare Card
  • Medicaid Card
  • Social Security Card
  • Insurance Cards
  • Power of Attorney papers
  • Long-Term Care Insurance Information
  • Living Will
  • Health Care Power of Attorney papers
