Situated in the serene farmland area in the heart of Putnam County, residents enjoy a peaceful country environment at Putnam Acres Care Center.
Established in 1866 by the residents of Putnam County, the facility offers a variety of living accommodations.
Providing progressive long-term care and short-term care, Putnam Acres Care Center is a dually certified Medicare/Medicaid nursing home just outside of Ottawa Ohio.

In 1866 the residents of Putnam County voted for a new "infirmary," which became the third county home to be built to care for the residents of Putnam County. This home was built in 1869 and residents moved in shortly thereafter. The County Infirmary continued to house residents of Putnam County until 1968. Only July 2, 1966, the Putnam County Commissioners began consideration for a new county nursing home. The Putnam County Commissioners, by resolution, stated that a levy to pay for the construction of a new county nursing home was "favorably passed."
Construction of the present county home began on July 12, 1967, with the digging by a Kalida firm. In January of 1970, the new Putnam County Home, now known as Putnam Acres Care Center, was completed. In November 1974, the Putnam Acres Care Center received certification as an intermediate care facility and became Medicaid certified. In September 1993, the nursing home became Medicare certified.
